The purpose of this form is to contest one or more violations on the citation/complaint number indicated above and request a civil hearing be scheduled. Please contact the court if you have any questions regarding this process. The court will consider this a "Not Responsible" plea and schedule a hearing for all violations on your citation unless you indicate otherwise in the "Information" box at the bottom of this page.
By submitting this request, you are accepting notification of the hearing date by email or mailing address above. If you fail to appear on the hearing date and time, a default Judgment will be entered against you with a responsible plea being reported to Arizona MVD, fines and fees will be imposed, and your case may be referred to a collection agency.

By requesting a hearing, I understand I am waiving the right to attend defensive driving school (only pertains to violations that are eligible for defensive driving school). This document must be received by the court no later than the "Appear By" date on your citation or if you have been given an extension, by the "Arraignment Extended" date.
